We accept new patients. Please contact the office to schedule a new patient appointment. Please bring your insurance card, government-issued identification card, current medications list with dosages, and pharmacy information to your appointment. If your insurance requires a co-pay, it will be collected at the time of your visit. We encourage you to sign up for our Patient Portal at your first appointment.
Our practice uses an electronic medical record system. You can use the portal to interact electronically with the practice for messages, scheduling, bill payment and other functions. You are also able to view components of your medical record. To request portal access, please contact the practice.
If you have provided your insurance information at the time of your visit or during registration, we will collect any co-payment from you and bill your insurance company for your treatment. If there is any remaining balance, we will send you a statement indicating the balance due. You may pay the balance via the Patient Portal or via mail. If you have questions about your bill or, if you would like to develop a payment plan for your balance or co-insurance portion, please call our office 520-638-2222.
